Net-30 credit application form — for new B2B customers asking for terms

When a new B2B customer asks for net-30 (or longer) terms instead of paying on receipt, you need basic credit information before extending the line. This form captures the entity details, asks for trade references, and includes the consent language you need to legally pull a business credit report and (with the owner's signature) a personal credit report on the principal.

Use it for

  • New B2B customers requesting net-30, net-45, or open-account terms over your trade-credit threshold (typically $1,000+).
  • Sole proprietors and young LLCs where the personal credit of the owner is part of the credit decision.
  • Building a paper trail of authorized credit pulls that satisfies FCRA permissible-purpose requirements.
  • Standardizing the new-customer onboarding so the credit decision is consistent customer-to-customer.

Do not use for

  • Existing customers with established payment history — you already have data on them.
  • Customers paying on receipt or by deposit — no credit is being extended, no application needed.
  • Public companies and large enterprises where credit is rarely a concern — request a vendor packet from their AP department instead.

SECTION 6 — PERSONAL GUARANTEE AND PERSONAL CREDIT CONSENT (REQUIRED FOR SOLE PROPRIETORS, AND FOR OTHER ENTITIES WHERE THE OWNER WILL PERSONALLY GUARANTEE)

SECTION 7 — TERMS OF EXTENSION

If approved, credit will be extended subject to {{provider_name}}'s standard terms and conditions, the law of {{governing_state}}, and the following:

(a) Invoices are due in full by the date stated. Past-due invoices accrue interest at the rate stated in the underlying agreement.
(b) {{provider_name}} may modify or revoke the credit limit at any time on written notice.
(c) If any invoice remains unpaid more than thirty (30) days past due, the personal guarantor (if any) becomes immediately liable for the unpaid amount.
